Use this form to join ACMS PTA for 2024-25 if you prefer to pay with check or cash (to join online, click here). Make checks payable to ACMS PTA. Please drop form and payment at the ACMS front office. PTA Membership is valid from August 1 to July 31 and must be renewed yearly.
PTA Funds Request Form (All In One)
Use this form for all reimbursement requests & vendor payment requests. Be sure to include your receipts in full and remember not to include sales tax in your request.
